PRIOR TO WORK, CLIENT MUST FORWARD A CERTIFICATE OF INSURANCE TO PMES.
INSURANCE REQUIREMENTS
Client will maintain at all times during the Agreement with carriers rated A-VII or better by A.M. Best
Company, the insurance below with limits not less than those stated (the “Required Insurance”).
Client will name Contractor Indemnitees as additional insured on the Required Insurance excluding
Workers’ Compensation. Client’s Required Insurance will be primary to any insurance of Contractor
Personnel. Client will, to the extent of the obligations assumed by Client, provide a waiver of
subrogation in favor of Contractor Indemnitees for the Required Insurance. Client will provide
Contractor with written notice of termination the Required Insurance not less than twenty days prior
to the effective date of termination. Client will be solely responsible for all Required Insurance
premiums and for deductibles and self-insured retentions. Client agrees that no deductible or self-
insured retention for the Required Insurance will exceed $50,000 without Contractor’s consent.
(a) Workers’ Compensation in an amount not less than the applicable statutory minimums.
(b) Employer’s Liability insurance in an amount not less than $1,000,000 for each accident as
respects bodily injury and in the aggregate as respects occupational disease.
(c) Commercial General Liability insurance, with a combined single limit of $2,000,000 for each
occurrence for bodily injury and property damage. Coverage shall include premises and operations,
independent contractors, contractual liability, broad form property damage coverage, and products
and completed operations coverage.
(d) Automobile bodily injury and property damage liability insurance with a combined single limit
of not less than $2,000,000 for each accident for bodily injury and property damage. Coverage
extends to owned, hired and non-owned automobiles.
The foregoing insurance coverage limits may be satisfied by a combination of primary and excess
coverages.
